We welcome new patients to Uppingham Surgery.
We do not accept patients from outside of our Practice area. However, we will consider registering patients who live at least 50 miles from our Practice boundary and who commute to work in our practice area on a regular basis.

How to Register
To register with our practice please complete our online registration form.
- After you have completed and submitted the forms you must bring the form to the surgery within 7 days with 2 forms of identification (ID).
One of these must contain your address (e.g. tenancy agreement, mortgage agreement, bank statement), and the other to have your photograph (eg. driving licence, passport, for children a birth certificate is adequate).
Until you have submitted your ID you will not be registered at the practice and cannot be seen for any treatment.
- If you have your NHS number, please include this on the registration form. It does not matter if you do not have it but it may help us to trace your medical records more quickly.
- You will be allocated a specific GP who has vacancies on their list, or who covers the area that you live in. If you have a preference please speak to the Patient Services staff member. However, if the doctors list is full, it may not be possible to accommodate your preference.
New Patient Health Check
All new patients aged over 16 are invited to make an appointment for a routine health check with a nurse or healthcare assistant. If you are taking regular medication or have any significant medical conditions we advise you to book an appointment with your new doctor.
We have four surgery sites. The main site is located in Uppingham. The branch surgeries are located in the villages of Barrowden, Gretton and Ketton.
Appointments are available at all sites with either a doctor or a nurse and prescriptions can be dispensed at all sites.
